1. Health & Safety 健康和安全　

1.1 Is there a clear Health and Safety Policy?

 能否有清楚的健康安全方針？         　     　

1.2 Are health and safety training records kept? Such as training for the hazards associates with their job, PPE, and safe use of machinery etc.

 工廠能否保管健康和安全練習記載？例如作業(yè)風險練習，個人防護用品練習，機械安全操作練習等等.     　     　

1.3 Are hazardous activities / substances storage and usage comply with the local legislation?Are the risk of harm from hazardous substances/activities minimised through the provision of safety equipment, signage, training and a safe factory infrastructure.

 風險工序和風險物質儲存和運用能否符合當?shù)胤▌t規(guī)矩？能否通過配備安全設備，標志，練習，和安全的處理準則等來降低風險物質和工序的危害和風險？

1.4 Is there evidence of using on site of necessary PPE?

2. Hygiene 清潔

2.1 Does factory provide a clean and hygienic place of work and rest consistent with local standards and the nature of the industry?

 工廠能否供應員工清潔和清潔的作業(yè)休憩場所，且與當?shù)貥藴屎捅韭殬I(yè)出產(chǎn)特性一起？     　     　

2.2 Do policies exist restricting smoking to named areas?

 能否有方針規(guī)矩，吸煙要到指定區(qū)域？         　     　

2.3 Do workers have access to drinking water throughout the working day?

 作業(yè)時，工人能否可以隨時取用飲用水？     　     　

2.4 Do workers have access to sufficient clean and hygienic toilets and washing facilities?

 員工能否具有數(shù)量滿足、清潔、清潔的廁所和沖刷設備？

3.3 Are accommodation areas clean and hygienic?

 宿舍區(qū)域能否清潔和清潔?         　     　

3.4 Is accommodation reasonably comfortable and provide for workers basic needs? does it allow for individuals privacy? This includes providing a place to store their personal belongings safely and securely(e.g. personal locker)

4. Fire Protection (Factory & Accommodation)  

 4.1 There must be an adequate number of safe, unblocked fire exits, escape routes and fire fighting equiment accessible to workers from each floor or area of the factory and accommodation (if provided)?

 工廠出產(chǎn)區(qū)域和宿舍（若供應）每層的防火出口、渙散通道須充分、安全、疏通，消防設備應滿足，適用。     Y/N     　     　

4.2 Are all fire exits and escape routes clearly marked in a language workers understand or shown graphically?

 能否悉數(shù)的安全出口和渙散路途都以員工了解的文字或圖形了解地標明?     　     　

4.3 Are workers trained in the use of fire fighting equipments and factory evacuation procedures?

 有否練習工人運用救活設備和逃生渙散方法?　     　

4.4 Are regular fire drills held ( at least once every 6 months)? And records of fire drills kept?Please give date of last drill?

4.5 The factory is a safe and hygienic place to work?

1.4 Does the factory have an Environmental Action Plan against which progress is monitored?

     能否有改進環(huán)境影響的舉動計劃？         　

1.5 The factory does NOT knowingly contravene local and national environmental legislation without being able to demonstrate a plan of action to improve?

     工廠沒有成心違反當?shù)睾蛧�家的環(huán)境法則需要，若有應能提出改進的舉動計劃。    　

1.6 Does the factory monitor energy usage?eg. Electric, fuel,etc.

2.1 Does the factory understand the issues associated with the provision of independently certified timber (in particular F.S.C.)?

     工廠能否了解與木材收買相關的，需證明木材合法來歷的疑問(尤其是經(jīng)F.S.C.森林處理委員會證明過的合法來歷木材)? 　     　

2.2 Does the factory have a ‘Chain of Custody’ in place which meets F.S.C. requirements?

     工廠能否有一個與F.S.C.森林處理委員會需要相符的”監(jiān)管鏈”?
